Request a Transcript

Transcripts are available from the registrar at the PHS Counseling Office in the 20s Building from 7:30am to 3:30pm Monday through Friday.
To obtain a transcript via mail, please send a signed request that includes:

- your name while attending PHS
- your date of birth
- year of graduation
- where you want the transcript(s) sent
The first TWO transcripts are free. Starting with the third transcript request, copies are $5 each, payable by check or cash. We cannot accept credit card payments. Please email check, made payable to Piedmont High School, to:

Please Note: We cannot accept requests from a second party without the written consent of the student. This includes parents of students 18 years or older.
