Request a Transcript
You can obtain a transcript at any time from the registrar at PHS. It is not necessary to phone ahead, but it can be helpful to do so. The registrar is located in the counseling office behind the PHS Library. Hours are 8:00-3:30 Monday through Friday.
If you are not located near PHS and need to obtain a transcript in the mail, please send a written signed request including:
- your name while attending PHS,
- date of birth,
- last year of attendance
- date of graduation and
- where you want the transcript(s) sent
Please enclose a check (payable to PHS) for $5.00 for each official transcript or $2.00 per unofficial copy. We cannot accept credit card payments. The address is:

Please Note: We cannot accept requests from a second party without the written consent of the student. This includes parents of students 18 years or older.
