Enrollment

Millennium High School takes several students from areas near to the City of Piedmont each year. These students are considered out-of-district students. Once accepted into MHS, these students must be able to submit an Inter-district Transfer Permit from their home school district to the Piedmont Unified School District. Application and Enrollment Forms and Procedures:

  1. Complete the Enrollment Form
  2. Submit a copy of your student’s birth certificate
  3. Submit current immunization records for your student
  4. Submit your student’s current transcripts
  5. If you have individual letters of recommendation, please submit  them with your packet as well.
