Millennium High School takes several students from areas near to the City of Piedmont each year. These students are considered out-of-district students. Once accepted into MHS, these students must be able to submit an Inter-district Transfer Permit from their home school district to the Piedmont Unified School District. Application and Enrollment Forms and Procedures:
- Complete the Enrollment Form
- Submit a copy of your student’s birth certificate
- Submit current immunization records for your student
- Submit your student’s current transcripts
- If you have individual letters of recommendation, please submit them with your packet as well.
