California Healthy Kids Survey – 5th, 7th-12th Grades

5th Graders, and 7th – 12th Graders will be asked to take the California Healthy Kids Survey at school. The survey includes questions about alcohol, tobacco, and other drug issues, as well as questions about school safety and the learning environment. Student participation in the survey is voluntary. We hope that you will encourage your child to participate so that his or her opinion and knowledge can be used to improve the school.

The survey is anonymous. No names or any other identifying information is connected to the answers except for the name of the school.

Elementary- Core only English and Spanish
Middle School – Core English and Spanish
Cal-Well – Middle and High School
Closing the Achievement Gap – Middle and High School
Drug-Free Communities – High School
Sexual Attitudes & Sexual Behavior Module— High School
Parent Survey – District-wide
California School Staff Survey – District-wide
